DMS Identification Badges
Every DMS employee, volunteer, intern and contract employee should have a current DMS identification badge. This badge should be worn at all times when on state business and provided to security to gain access to various office locations.
Need a new badge?
Identification badges are provided to new employees during New Employee Orientation (NEO). If you begin work on a day prior to NEO, your sponsor should schedule an appointment to have your badge made on your first work day.
Identification badges are made in the DMS Human Resource (HR) office during the following hours. If this time is not convenient, contact the HR team at (850) 488-2707, to schedule an appointment. You will be requested to complete this form and show a picture ID if you are not personnally known by the HR staff: DMS Identification Badge Request (msword 142.00 kB) .
Location: CCOC, Building 4050, Room 260H.
