Governors Commission on Disabilities Public Comment Form

This form is to be used for public comments/ feedback to the Commissioners by the public. Individuals wishing to attend the meeting and/or address the Commission meeting should complete this form.

If you have any materials to include in the meeting packets for the Governors Commission on Disabilities, please send copies to Jamie Bullock (jamie.bullock@dms.myflorida.com). The materials will be distributed to all Commissioners with their meeting materials.

Time allotment for public comment will be determined depending upon the number of those expressing interest.
